Effective navigation is important to providing coordinated person-centred care and support. Care navigation is an emerging idea in the UK, with a growing evidence demonstrating benefits for patients and carers.
Care navigators can occupy many roles and play a crucial part in helping people get the right support, at the right time, to help manage a wide range of needs.
Our Reception team will now be called Health Navigators and will ask you when you request an appointment if they can ask you why you want that appointment so they can direct you to the best person. Please do not be offended by this. All information you give us is treated in confidence.
We have a variety of Health Professionals here including a Clinical Pharmacist and Nurse Practitioner here to help you.
