New Patient Registration
The doctors welcome new patients who live within our practice area. As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire.
Medical treatment is available from the date of registration. Please contact reception for further information.
Please note that we do not register patients who live outside our Practice Area.
Each patient regardless of age must complete a GMS1 form. Please ensure that you include your NHS number (your previous GP surgery will be able to provide you with this) and sign the form.
We ask that all new adult patients (over 16 years) also complete our New Patient Health Check Questionnaire.
If you wish to register for online services, please provide a copy of photo ID (either a valid passport or driving licence).
For patients moving to the area on a temporary basis of up to 3 months, please complete a GMS3 form.
Please return completed forms to us either by email or post.
